Fantasy abounds, from complete cell genesis to sex-changes at the turn of a dime. More mundane applications should be very useful, however, such as the example given by Platt of nano virus killers and the like. I would also anticipate later nanotech that could enter cells and correct DNA problems due to aging and damage, and clean out cell debris, these capabilities would make us essentially ageless as we wait for more improvements in nano. The book TRAVELS TO THE NANOWORLD is a good book to read, it points out the use of biological components in nanotech, and a recent posting here about the work with E-Coli bacteria is very interesting.
