As for the first one, if the nuclear rockets are all made in space, half the problem is solved. It's not clear that it would need a nuclear rocket anyway ... sure, that would carry lots of waste, but smaller amounts (which relative to coal plants nuclear plants make) could be easily handled either by devices shooting them into space or hydrogen-oxygen fueled rockets.
