One detail that hasn't been mentioned in this discussion
and which is fairly easily implemented in these days of
affordable digital camcorders is filming the entire
process, starting with you stating with your intentions,
and anyone who will be involved stating their part in
the process.  This will probably want to be scripted in
advance, perhaps with an cryonics-friendly lawyer as a
consultant.
