A number or years back I put together a "Personal  Profile of ______" book.
There are 12 pages with personal information blank spaces.  I have included
the opening page here and if you are interested send me an email request and
I will send a copy of the Personal Profile Book for you to fill in your own
blanks.

The Chamberlains also did a Lifequest self interview video script, that you
can use to make your own video documentary.

The purpose of this personal profile is to record in one place

the history of what I have been and what I am now, what I think and what I
feel about myself and my surroundings.



 Through out life we have been in a constant state of change and with some
luck growth.  What ever our minds have recorded and the way we use this
information in our living experience is what we are.  With each moment we
breath, with each moment we are aware, we strengthen what we are. The
essence of what we are at the moment of deanimation is what we want to have
at the moment of reanimation.



 This personal profile will attempt to capture the essence of ME and serve
as a bench mark to help recreate a full set of memories upon my reanimation.




Some memories may be present, but inaccessible after reanimation due to the
CRYOGENIC process. Sometimes just the smallest detail may be enough to
reestablish the correct path to previously inaccessible memories.  Therefore
it is important to include all pertinent information and continue to update
it as you think of more things about your self.  It is conceivable that
after brain cell repair with the use of nanotechnology that memories
unavailable even today will be available upon reanimation.
