Hello, Fellow Cryoneters:
With thanks to Bill Faloon, I have provided the following instructions on a
single sheet of paper.
This simple document will be carried with me, with a copy framed in my
office, a copy in my car, and a copy with my sister.
It may not be perfect. But, in conjunction with the "Emergency
Instructions" in my wallet, and the signed card with a picture declaring my
"religious objection to autopsy", there is a better chance of a people doing
the right thing if something happens and I "die".
I am providing it here on cryonet because I think that this is a good idea.
It is shocking that even long term cryonics advocates are still getting
suboptimal (and sometimes downright awful) suspensions due to variables that
SHOULD and MUST be anticipated and controlled. Sorry if this sounds ranty,
but I have recently read the actual histories of suspensions very candidly
displayed on both the Alcor and the Cryonics Institute websites.
Jesus, what a wake up call these are!
While the heroism of cryonics activists and management seems apparant to me
in these tales, many of these are cautionary tales we would do well to
re-read so we can make smarter decisions and arrangements than our
predecesors.
Anyway, I hope this document translates to the cryonet format without too
much confusion, and can help. This can be reworked for your cryonics
organization(s) and arrangements.

My name is Rudi Hoffman and I have executed a donor document with Alcor Life
Extension Foundation of Scottsdale, Arizona in conformity with the Uniform
Anatomical Gift Act, in support of my very serious desire to be
cryopreserved after legal death.
To this end, I wish to be protected from ischemic injury and other forms of
biochemical damage as rapidly as possible after my death has been
pronounced.
If representatives of Alcor Life Extension Foundation are not present,
representatives of *Suspended Animation, Inc.* should be permitted to deploy
appropriate medications and equipment and intervene in any way that will
further my wish to be cryopreserved with minimal injury. The contact

arrangements and has agreed to expedite any logistics required by Alcor
and/or Suspended Animation to effect the best possible cryotransport.
3. My doctor, who also understands my cryonics arrangements, is Dr.
Christopher Larazabal. His office number is 386-676-2779.
4. Also, please call my wife Dawn Hoffman at home number *386-788-3773*.
5. Also, my sister and personal representative Trudi Taylor at cel *

7. While the above calls are being made, cool my head and body with water
ice, while continuing CPR. Continue CPR and cooling even AFTER I am
pronounced legally dead, to reduce ischemic damage to my brain.
If representatives of Alcor are present, representatives of Suspended
Animation, Inc. should still be permitted to participate in any way they
feel is appropriate.
