When he was critically injured in an accident, no documentation confirming his wishes could be found. Meanwhile John lingered in a coma, unable to take any action or express any wishes. Finally he died. He was not frozen. Moral: *if you want to be frozen, sign up!* The signup process, however, is involved and tedious (a lot of legal documents must be executed, and details of funding must be arranged) and it puts off many. Many in fact, who are on the verge of carrying it out, *still have not signed a single document expressing a wish to be frozen.* It is, of course, best by far to complete your paperwork with the organization of your choice and finalize your arrangements. But failing that, *it is at least desirable to execute some legal document establishing your wish to be frozen, should the need arise.* Included below is a "Declaration of Intent" form expressing the wish to be preserved, in event of death, through cryonic suspension, and revoking any contrary provisions previously made. It is a simple one-page document that can be easily executed without any financial obligations. It should be understood *that this is not a contract to be suspended and will not in and of itself get you frozen.* Arrangements must still be made with a cryonics organization if you are to be suspended. The document, though, could be useful in case you haven't made formal arrangements and death intervenes, particularly if you are so fortunate as to have someone else who is willing and able to complete the arrangements. It could also be useful for friends or acquaintances you'd like to see covered. It is *not* the best way to proceed, but it *is* better than nothing.
